After completing all the puzzles/challenges in the forest vault, when I arrive at the huge cave at the end where the boss is supposed to be he does not appear. So I am unable to complete the vault or receive any of the rewards. I have attempted the forest vault 3 times now and this has happened on all 3 occasions.

Thank you for bringing this to our attention.
To clarify, the boss of the Forest Vault is an Elder Eoten, and will not combat the player until attacked first.
Was there a giant tree present in the final chamber of the Vaults you visited? If there was, did you attempt to strike it?

I did not know that you had to strike the tree in the middle. I went back and did that and the boss appeared.
