Freezing screen when dying & survivor not reviving

Sometimes when I die and press give up, the screen freezes on the "memento mori respawning in 5 seconds" screen. I have to exit the game and re-enter to fix it. I will have to do this when my survivor does not revive me. I have seen a few times if his equipped weapon breaks he will not revive me. Also, even if his weapon is perfectly fine, he will sometimes just stand there and not revive me.
