It would be a series of joined modules somewhat like how the river and lake generator works that will chain together and pop out at the existing cave POIs. The existing POIs would have a rockface or buried ruined door at the back that could randomly be removed and become an entrance to the large cave system during generation.
Or even just a set of stairs going into the earth in a building. Might be a challenge for swamps :)
