It is always in a 'Bastille of Might' inside Forest Vault, inside the biggest room there is. Usually they spawn in/behind a wall (below) near the exit (on top), furtherest away from the 'blue button' to launch the wave of attack.
Thank you for bringing this issue to our attention. If at all possible, please press F2 while in-game in one of these vaults and attach a screenshot of the overlay menu to this post so we can better understand what maybe causing the issue.
